Our Policies

Payment

Payment is due upon receipt of services. We will provide you with a receipt to submit to any private insurance that you may have. We are happy to assist you with any other documentation that you may require to submit a claim to your private insurance.

Prescriptions

All prescriptions are valid from one year from the date of issue. Due to quality control and our desire to ensure accuracy of all prescriptions filled by our office, we do not fill any glasses or contact lens prescriptions generated outside of our office. If you have had a recent exam, we are happy to verify the accuracy of the prescription during a brief appointment. Upon request a copy of your prescription will be provided to you. Measurement of Interpupillary Distance is not required to determine your refractive error, therefore it is not part of the prescription. PD measurements are to be determined by the provider of eyewear.

Ordering

We require 50% down on a glasses order to initiate lens production. Frames and lenses must be paid for in full at the time of dispensing. We request that contact lens orders be paid in full at the time of ordering.

Progressive Lens Warranty

Our progressive lenses have a 45 day non-adapt warranty. If you cannot adapt to or are unhappy with your progressive lenses, they can be remade into lined Bifocals, Trifocals, or Single Vision lenses at no charge.

Cancellations/Missed Appointments

We request 24 hour notice to cancel an appointment. We reserve the right to charge a missed appointment fee for cancellation of appointments with less than 24 hours or for missed appointments. We may request pre-payment to book future appointments. Continued failure to keep an appointment or repeated cancellations may result in your dismissal as a patient.

Privacy Policy

All patient information is confidential.  All requests for information must be accompanied by a signed release from the patient or the patient's legal guardian.  Third party requests may be verified by our office.  Fees may apply to copy or transfer your file.
